Helan Health Insurance contributes to the cost of a sports medical examination. You receive a 25 euro reimbursement per year for a preventive sports check-up.
Download the form and have your doctor fill it in.
Download the form for sports examinations and have it completed by your (sports) physician or medical specialist.
Send the completed form to Helan. Don't forget to include your adhesive stamp.
- Digitally: via the Helan app or via My Helan.
- By post to Helan Health Insurance, Boomsesteenweg 5 in 2610 Wilrijk, with a white adhesive stamp. Or drop it in a Helan mailbox near you.
We will transfer the amount to your bank account.
- You are up to date with your health insurance contributions.
- Only for examinations such as a sports examination, fitness test, maximum effort test or movement analysis, for which statutory health insurance does not intervene.
- The examination must be performed by a (sports) physician or medical specialist.
- This benefit does not apply to blood tests, mental coaching or nutritional analyses.
- This benefit only applies when performed in Belgium.
- This reimbursement is not cumulative with the benefit preventive examinations for the same treatment.
