How can we help you?
Organising home care is not always easy. During our multidisciplinary consultation, everyone involved sits down together to map out the current situation and explore where improvements can be made. In collaboration with i-mens.
- Together with all parties involved, clear agreements are made and recorded in a report. This ensures everyone knows who will visit the home and for which type of support, allowing care to be better coordinated and tailored to individual needs.
- If the situation changes, a new consultation can be scheduled.
- Any party involved can request a home care consultation through our Social Work department.
Good to know
Does your partner, a family member or a neighbour help you with your administration? Or are you in need of care and does an informal carer manage everything for you? Ask your informal carer to register as your contact person. That way, we can quickly move forward with any questions about your file.
Apply for home care coordination
Would you like a multidisciplinary team to review your home care situation together? You can apply easily.
Complete the contact form and select ‘Social Work department’. Let us know that you would like to request a multidisciplinary consultation. Please include the following details:
- name and contact details of the applicant
- name, address and national registration number of the person in need of care
- name and telephone number of the general practitioner
- name and telephone number of the care services already involved (cleaning help, home nursing, home help, etc.) and their weekly frequency
- name and telephone number of family members and informal carers who wish to attend
- name and telephone number of any other people you would like to invite
- the reason for your request and a brief description of the situation
- your expectations of the multidisciplinary team
Our social worker submits the application for a home care consultation through a multidisciplinary team to i-mens.
I-mens takes care of the practical organisation. They process your application and details and take the necessary steps to bring everyone together around the table.
I-mens will inform you when the multidisciplinary team is meeting. You are, of course, welcome to attend. Together with you, they will fine-tune the home care plan.
