Proof of affiliation to health insurance fund

• In the menu on the left, go to ‘Documents’.
• Click on the ‘Search documents’ option.
• Then select section ‘08. My health insurance fund file’.
• Then search for ‘Declaration of membership’.

• Choose for whom you want to request the document.

• Download the document.
