Register a contact person

Does someone manage your administration? Then you can register this person as a contact person. For this, we need your permission. Please note: you can only register one contact person. By registering a contact person, they will have access to your health insurance details, including your medical information. However, the contact person cannot make changes to your file.

How do you complete the document?

The top section must be completed by the person granting the authorisation.
The bottom section must be completed by the authorised person.
The person granting the authorisation signs at the bottom.

Example: Would you like access to your father’s file to manage his administration? Then your father must sign the document as the person granting the authorisation.

Sign digitally

Download the power of attorney document.
Open the file and complete the form in full.
Connect your eID card reader to your computer to complete the field 'signature of the person granting the authorisation'.
Insert your identity card into the eID card reader.
The system will complete the document based on your identity card.
Click ‘sign’ to sign your document.
Your computer will ask you to save the document.
Enter the PIN code of your identity card, save the signed PDF on your computer and close the document.

Or complete it on paper

Download the power of attorney document, print it, complete it and sign it.
Make a copy of your identity card (front and back) and add this copy to your request.
